Evaluating the Efficacy of a Safety Algorithm for Laparoscopic Access in Severe Acute Appendicitis
Laparoscopic appendectomy for complicated forms of acute appendicitis remains a subject of debate due to concerns related to the high incidence of intra-abdominal abscesses. The aim of this study was to evaluate the effectiveness of a developed “Safety Algorithm” incorporating standardized risk stratification criteria, intraoperative irrigation protocol, and dynamic postoperative monitoring. A retrospective analysis of data from 81 patients with destructive forms of acute appendicitis (ICD-10 code K35.2 – acute appendicitis with generalized peritonitis) treated during 2024–2025 using laparoscopic approach according to the authors’ algorithm. The study evaluated hospitalization duration, conversion rate, postoperative complications, dynamics of inflammatory markers, and outcome structure. The Safety Algorithm, based on objective severity stratification (WSES scale), standardized irrigation technique, and “Red Flags” dynamic monitoring protocol, enables laparoscopic appendectomy even in cases of diffuse peritonitis with zero conversion rate, minimal mortality, and reduced hospitalization duration. This standardized approach demonstrates reproducible results independent of individual surgeon experience, offering a transformative paradigm for managing complicated acute appendicitis through evidence-based risk stratification, comprehensive intraoperative debridement protocols, and structured postoperative surveillance utilizing objective inflammatory biomarkers.
