В последние годы установлены новые генетические механизмы, детерминирующие развитие наследственного ангиоотека. Дефицит С1-ингибитора (истинный или функциональный) приводит к неконтролируемой активации калликреин-кининовой системы с образованием брадикинина, являющегося основным медиатором наследственного ангиоотека. В связи с этим калликреин является одной наиболее значимых мишеней для новых таргетных лекарственных средств-ингибиторов. Ланаделумаб, ингибитор калликреина, был использован для долгосрочной профилактики обострений у пациентки с длительным анамнезом наследственного ангиоотека и показал достаточную эффективность и безопасность, что согласуется с данными многочисленных крупных многоцентровых исследований.</p></abstract><trans-abstract xml:lang="en"><p>The purpose of this work is to review the results of modern studies on the effectiveness and safety of lanadelumab, a new drug for biological targeted therapy of hereditary angioedema. The article also presents the results of our own experience in clinical use of this option for long-term prevention of angioedema attacks. In recent years, new genetic mechanisms have been identified that determine the development of hereditary angioedema. C1-inhibitor deficiency (true or functional) leads to uncontrolled activation of the kallikrein-kinin system with the formation of bradykinin, which is the main mediator of hereditary angioedema. In this regard, kallikrein is one of the most significant targets for new targeted inhibitor drugs. Lanadelumab, a kallikrein inhibitor, was used for long-term prevention of exacerbations in a patient with a long history of hereditary angioedema and showed sufficient effectiveness and safety.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>наследственный ангиоотек</kwd><kwd>калликреин</kwd><kwd>эффективность и безопасность</kwd><kwd>ланаделумаб</kwd></kwd-group><kwd-group xml:lang="en"><kwd>hereditary angioedema</kwd><kwd>kallikrein</kwd><kwd>efficacy and safety</kwd><kwd>lanadelumab</kwd></kwd-group></article-meta></front><back><ack><p>Информированное согласие.
