Методы визуализации в диагностике гипертрофической кардиомиопатии
Обзор посвящен методам визуализации сердца в диагностике, лечении и стратификации риска внезапной сердечной смерти у пациентов с гипертрофической кардиомиопатией (ГКМП). При работе с англо- и русскоязычными источниками использовались поисковые запросы по ключевым словам в базах данных e-Library, PubMed, Scopus и Web of Science за период с 2014 по 2023 гг. В обзоре представлены возможности эхокардиографического исследования в оценке наличия, распределения и выраженности гипертрофии миокарда, состояния митрального клапана и подклапанных структур, обструкции выносящего тракта левого желудочка, систолической и диастолической функций. Обсуждается роль компьютерной томографии, обладающей лучшими возможностями в более точной оценке толщины миокарда, анатомии коронарных артерий с идентификацией «миокардиального мостика». Подробно представлена магнитно-резонансная томография (МРТ) в диагностике и ведении больных ГКМП. Благодаря МРТ удаётся выявить различные фенотипы заболевания за счёт получения изображений высокого качества с резким контрастом между миокардом и кровотоком. Среди преимуществ метода МРТ отмечена возможность получения детализированных тканевых характеристик миокарда, что позволяет визуализировать очаги фиброза. Отмечено значение других методов визуализации, в частности однофотонной эмиссионной томографии, сцинтиграфии миокарда и коронароангиографии, в ведении больных ГКМП.
